Please complete this form fully to be considered for admission. Note that applications received prior to April 15 will receive priority attention. Applications received after April 15 will be reviewed on a space-available basis.

What to submit with your application: a $20 non-refundable application fee, ACT or SAT test scores, your official final high school transcript or GED results, and official final transcripts from all previous colleges attended. A personal essay may be requested after transcripts and scores are received. The application fee can be paid by credit card; call 1-800-779-7366.
