Please complete this form fully to be considered for admission. Note that applications received prior to April 15 will receive priority attention. Applications received after April 15 will be reviewed on a space-available basis.
Please submit the following with your application:
- ACT or SAT test scores
- Official final high school transcript or GED results
- Official final transcripts from all previous colleges attended, including credits earned as dual credit in high school (if applicable)
Note: A personal essay may be requested after transcripts and scores are received.
